Agent context
Incidents track things like pause events (GET/PATCH /v1/ops/incidents).

molted incidents list --status open
molted incidents ack <id>
molted incidents resolve <id>Resolving an OPEN PAUSE incident resumes the account, the same human decision as molted resume (D5): an agent gets 403 human_decision_required with a decisionUrl, unless the tenant's human-gate mode is "warn" or "off". Acknowledging an incident does not resume the account and is never gated.
